To get you started we will need a copy of:

• Valid photo ID (State or Government Issued)
• Insurance card (if you have insurance)

For those who have any insurance that we accept, co-pays and/or deductible amounts will be due at the time of the visit. For self-pay patients, please inquire with your practice of the fee schedule. We only accept cash and major credit cards as a method of payment. No checks, gift cards, or money orders of any kind. No exceptions.
